Apcalis SX Oral Jelly (tadalafil)
The practical difference between this and a standard tadalafil tablet comes down to two things: format and onset. The jelly absorbs faster than a tablet — effects begin in 15 to 20 minutes for most people — and the lack of required water means it can be taken discreetly in situations where that matters. The active ingredient, tadalafil, and everything downstream of that is identical to Cialis.
Each sachet contains 20 mg tadalafil. Available in various fruit flavors.
The 36-hour window
Tadalafil’s defining characteristic among PDE5 inhibitors is duration. A single 20 mg dose remains effective for up to 36 hours — substantially longer than sildenafil’s 4 to 6 hours or avanafil’s 6 hours. This doesn’t mean an erection lasting 36 hours; it means the pharmacological support for achieving one is available for that window. Many men find the lack of timing pressure the most meaningful aspect.
One sachet per 36 hours is the minimum interval. Don’t exceed one dose in that period.
Food doesn’t matter
Unlike sildenafil, tadalafil’s absorption is not affected by food. A high-fat meal before sildenafil delays and reduces peak concentration. With tadalafil — including this jelly formulation — you can take it with or without food, shortly before or after eating, and the effect is consistent. This is a meaningful practical difference for on-demand use.
How it works
Tadalafil inhibits PDE5, an enzyme that breaks down cyclic GMP in penile smooth muscle. During sexual arousal, nitric oxide is released, which produces cyclic GMP, which relaxes smooth muscle and allows blood to fill the erectile tissue. PDE5 normally limits how long this response lasts. Tadalafil prolongs it. Without sexual stimulation, the drug has no visible effect — it amplifies a physiological response, it doesn’t produce one independently.
Nitrates — absolute contraindication
Tadalafil cannot be combined with nitrates in any form. Nitroglycerin, isosorbide mononitrate, isosorbide dinitrate, nicorandil, amyl nitrate (“poppers”) — all are absolutely contraindicated. Both nitrates and PDE5 inhibitors dilate blood vessels; together they can cause a sudden, severe drop in blood pressure that can be fatal. There is no safe dose or timing interval that makes this combination acceptable.
Contraindications
Nitrates (as above). Recent heart attack within 90 days. Unstable angina or severe heart failure. Blood pressure below 90/50 mmHg. Severe kidney failure. A history of non-arteritic anterior ischemic optic neuropathy (NAION) — a specific type of vision loss associated with PDE5 inhibitor use, particularly in men with a small optic disc, diabetes, or hypertension. Hypersensitivity to tadalafil.
Side effects
Back pain and muscle aches are more common with tadalafil than with shorter-acting PDE5 inhibitors — typically appearing 12 to 24 hours after use and resolving within 2 days. Headache, facial flushing, nasal congestion, and digestive discomfort are the other common effects. These are more common at higher doses and usually mild.
Priapism — erection lasting more than 4 hours — is rare but requires emergency treatment. Prolonged erection causes irreversible ischemic damage to erectile tissue. If erection persists beyond 4 hours, seek emergency care regardless of whether it’s painful.
Alcohol
Moderate alcohol (up to 3 units) is generally compatible with tadalafil. Larger amounts increase vasodilation and dizziness risk, and reduce the quality of the sexual response through mechanisms unrelated to the drug.
