Caverta (sildenafil)

Caverta is a generic sildenafil manufactured by Ranbaxy, available in 50 mg and 100 mg tablets — the same active ingredient as Viagra at a fraction of the cost. The pharmacological profile is identical. The clinical guidelines for sildenafil consistently recommend starting at 50 mg: it’s effective for most patients, and if it isn’t, you have room to move up. Starting at 100 mg because it seems like more isn’t wrong, but it increases both the side effects and the cost per dose without necessarily improving outcomes.

How it works and when

Caverta-tabletSildenafil inhibits PDE5, the enzyme that limits blood flow to penile tissue. During sexual stimulation, the resulting vasodilation fills the corpus cavernosum. The drug prolongs the response; without stimulation, it has no visible effect.

Onset is typically 30 to 60 minutes. A high-fat meal before taking sildenafil delays and reduces peak concentration — lighter meals or taking it on an empty stomach produces more consistent results. Duration: 4 to 6 hours at standard doses.

Starting dose and adjustment

50 mg is the recommended starting point. If the response is insufficient, 100 mg is the maximum single dose — and for many men, the jump from 50 mg to 100 mg does make a meaningful difference. For men over 60 or with reduced kidney function (lower creatinine clearance), starting at 25 mg is advisable.

Nitrates — no exceptions

Sildenafil cannot be taken with nitrates in any form. Nitroglycerin tablets or patches, isosorbide mononitrate, isosorbide dinitrate, amyl nitrate (“poppers”) — all cause a potentially fatal blood pressure drop when combined with sildenafil. There is no safe window or dose that makes this combination acceptable.

Contraindications

All forms of nitrate therapy. Unstable cardiovascular disease — the physical exertion of sexual activity may be risky in patients with severe heart disease regardless of the drug. Severe hypotension or uncontrolled hypertension. Retinitis pigmentosa. History of NAION (non-arteritic anterior ischemic optic neuropathy). Sickle cell anemia, leukemia, or multiple myeloma (increased priapism risk). Peyronie’s disease. Severe hepatic impairment. Concurrent strong CYP3A4 inhibitors including certain antifungals and antiretrovirals — these significantly raise sildenafil blood levels.

Side effects

Headache, facial flushing, nasal congestion, indigestion, and mild visual disturbances (blue tint, increased brightness) are the most common and are dose-dependent — more pronounced at 100 mg than at 50 mg. These typically peak within an hour and resolve within a few hours.

Priapism — erection exceeding 4 hours — requires emergency evaluation. Permanent damage to erectile tissue occurs without prompt treatment.

Sudden hearing loss or sudden vision loss are rare but documented. Both require immediate discontinuation and same-day medical assessment.

Alcohol and grapefruit

Up to 3 units of alcohol is generally compatible. Larger amounts compound the hypotensive effect and reduce sexual performance independently of the drug. Grapefruit and pomelo juice inhibit the CYP3A4 enzyme that metabolizes sildenafil, raising blood levels unpredictably. Avoid both on days you take the medication.
