Malegra (sildenafil)
Malegra is a generic sildenafil citrate manufactured in India, available in 50 mg and 100 mg tablets. It contains the same active ingredient and works by the same mechanism as Viagra. The choice between 50 mg and 100 mg isn’t arbitrary — it’s a clinical decision that should start at the lower end.
Dosing
50 mg is the recommended starting dose for most men. Taken 40 to 60 minutes before sexual activity, on an empty or light stomach. If 50 mg produces adequate results, there is no clinical reason to escalate to 100 mg — higher dose means more side effect exposure for no additional benefit when the lower dose already works. If 50 mg is insufficient after a fair trial, 100 mg is the next step and the maximum. One dose per 24-hour period.
An empty stomach produces faster, more reliable absorption. A heavy meal — particularly high-fat content — delays peak plasma concentration by up to an hour and reduces it. Effects duration: 4 to 6 hours.
How it works
Sildenafil inhibits PDE5, the enzyme that degrades cyclic GMP in penile smooth muscle. Sexual stimulation triggers nitric oxide release, which generates cyclic GMP, which relaxes smooth muscle and allows blood to fill erectile tissue. Sildenafil extends that response by slowing cyclic GMP breakdown. Without stimulation, it has no visible effect.
Nitrates
Sildenafil combined with nitrates — nitroglycerin, isosorbide dinitrate, isosorbide mononitrate, amyl nitrate — causes severe and potentially fatal blood pressure drops. This is absolute. No dose or timing adjustment makes this combination safe.
Contraindications
All nitrates. Recent stroke (within 6 months). Significant heart failure or unstable angina. Uncontrolled blood pressure irregularities. Blood disorders predisposing to priapism — leukemia, multiple myeloma, sickle cell disease. Peyronie’s disease. Retinitis pigmentosa. History of NAION. Severe renal or hepatic impairment. Hypersensitivity to sildenafil.
Side effects
Nasal congestion, nausea, indigestion, and back pain are the most common at 50 mg and above. Visual disturbances — blue tint, brightness sensitivity — reflect PDE6 involvement in the retina and are dose-dependent. At 100 mg they’re more frequent. Hearing loss has been reported rarely.
Serious effects requiring same-day evaluation: chest pain, sudden vision loss, sudden hearing loss, severe dizziness.
Priapism — erection lasting more than 4 hours — is a medical emergency. Irreversible damage to erectile tissue occurs without prompt treatment.
Alcohol and grapefruit
Both potentiate the hypotensive effect and increase side effect risk. Grapefruit inhibits the CYP3A4 enzyme that metabolizes sildenafil, raising blood levels unpredictably. Avoid grapefruit juice on days you take the drug. Limit alcohol intake.
