Description
Cenforce (sildenafil)
Cenforce is a generic sildenafil manufactured by Centurion Laboratories in India. The active ingredient and mechanism are identical to Viagra. Where it differs from other sildenafil generics is the dose range: it starts at 25 mg and extends through 50 mg, 100 mg, and up to 120 mg, 150 mg, and 200 mg formulations.
That upper range deserves direct comment. The FDA-approved maximum single dose for sildenafil is 100 mg. Doses of 120 mg, 150 mg, and 200 mg are not covered by FDA labeling — they represent dose escalation beyond what has been evaluated in the regulatory process. Higher doses produce higher blood levels and a proportionally greater risk of side effects, including hypotension and cardiovascular effects. Anyone considering doses above 100 mg should do so only under medical guidance, not independently.
Starting dose
Clinical guidelines recommend 50 mg as the starting dose for most men, taken 30 to 60 minutes before sexual activity. If 50 mg is effective without troublesome side effects, there’s no reason to increase. If it’s insufficient, 100 mg is the next step and the upper boundary of labeled dosing. Begin with the lowest dose that works — not the highest available.
Food and timing
High-fat meals delay and reduce sildenafil absorption. A heavy dinner before a 50 mg tablet may produce substantially less effect than the same dose on an empty stomach. Duration of action is 4 to 6 hours regardless of dose.
Nitrates
Sildenafil cannot be combined with nitrates in any form. Nitroglycerin, isosorbide preparations, amyl nitrate — all cause potentially fatal hypotension when combined with PDE5 inhibitors. This is an absolute contraindication without exceptions.
Contraindications
Hypersensitivity to sildenafil. All nitrate medications. Severe cardiovascular disease or recent stroke or heart attack. Severe hepatic or renal failure. Retinal disorders including retinitis pigmentosa and history of NAION. Concurrent use of strong CYP3A4 inhibitors (certain antifungals and antiretrovirals significantly raise sildenafil blood levels).
Side effects
Headache, flushing, nasal congestion, digestive discomfort, and visual disturbances are the most common and are dose-dependent. They’re more pronounced at 100 mg than at 25 mg or 50 mg, and markedly more pronounced at doses above 100 mg.
Priapism — erection lasting more than 4 hours — is a medical emergency requiring immediate treatment. Delay causes permanent erectile tissue damage.
Interactions
Antihypertensive medications combined with sildenafil can produce additive hypotension — not an absolute contraindication, but clinically relevant. Grapefruit juice inhibits sildenafil metabolism. Alcohol at levels above 3 units compounds vasodilation and orthostatic hypotension risk.
