Megalis (tadalafil)

Megalis is a generic tadalafil manufactured by Macleods Pharmaceuticals in India. It’s available in 10 mg and 20 mg tablets for on-demand use, and in a 2.5 mg daily dose formulation. The 2.5 mg daily option reflects tadalafil’s longer half-life — approximately 17.5 hours — which makes it suitable for continuous low-dose maintenance rather than pre-activity dosing.

Dosing

For on-demand use: start with 10 mg, taken 30 minutes before sexual activity. If 10 mg is sufficient, stay there. If not, 20 mg is the maximum single dose. One tablet per 24-hour period regardless of strength.

For daily use: 2.5 mg taken at the same time each day, regardless of planned activity. This approach removes the timing requirement altogether — tadalafil maintains a steady state and responds to arousal whenever it occurs. For men who find the on-demand window stressful or who have frequent sexual activity, daily dosing is worth considering over the on-demand approach.

Tadalafil can be taken with or without food. Unlike sildenafil, high-fat meals don’t meaningfully affect its absorption. Duration of on-demand effect: up to 36 hours — longer than any other oral PDE5 inhibitor.

How it works

Tadalafil inhibits PDE5, prolonging the cGMP response to sexual stimulation in penile smooth muscle. Sexual arousal is required; tadalafil doesn’t produce erections independently. The mechanism is shared with sildenafil and vardenafil, but the pharmacokinetic profile — long half-life, food-independent absorption, extended duration — differentiates tadalafil in practice.

Nitrates

Tadalafil is absolutely contraindicated with nitrates in any form — nitroglycerin, isosorbide dinitrate, isosorbide mononitrate, recreational “poppers.” Combined use causes severe, potentially fatal blood pressure drops. This applies equally to daily and on-demand dosing.

Contraindications

All nitrates and recreational nitrites. Certain alpha-blockers require caution — hemodynamic stabilization on the alpha-blocker dose is required before adding tadalafil. Severe cardiovascular disease. Recent stroke or myocardial infarction. Uncontrolled hypertension. Retinitis pigmentosa. History of NAION. Severe hepatic impairment. Hypersensitivity to tadalafil.

Side effects

Back pain and muscle aches are distinctive to tadalafil among PDE5 inhibitors, related to PDE11 inhibition in muscle tissue. These typically appear 12 to 24 hours after dosing and resolve without treatment. Headache, indigestion, facial flushing, nasal congestion, and dizziness are shared with other PDE5 inhibitors.

Sudden vision or hearing loss, chest pain, and severe dizziness require immediate evaluation and cessation of the drug.

Priapism — erection lasting more than 4 hours — is a medical emergency. Permanent damage to erectile tissue occurs without prompt treatment.

Grapefruit

Grapefruit and pomelo inhibit CYP3A4 metabolism of tadalafil, raising blood levels unpredictably. Avoid both on days of dosing. For daily use, consistency matters more — avoid grapefruit juice altogether while using Megalis regularly.
