What is Clomid?

Clomid (clomiphene citrate, or clomiphene), is a structural analogue of estrogens, and inhibits their action by preferentially binding to the receptor of the hypothalamic neuron normally intended for the reception of estrogen hormones. This makes clomiphene a competitive inhibitor of estrogens at the level of hypothalamic receptors. Clomiphene can be used alone in combination with metformin to induce ovulation in infertile women by follicular insufficiency, corresponding to a failure of estradiol production by the ovarian cortex.

A form of infertility is treated in women (follicular insufficiency): the follicles stimulated will grow until only one of them, the follicle of De Graaf, expels the oocyte in the fallopian tube. It is part of the list of essential medicines of the World Health Organization (list updated in April 2013).

What is Clomid used for?

Clomid is used in the treatment of anovularia associated with alterations of the luteinic phase or due to oligomenorrhea, secondary amenorrhea and polycystic ovary. By inhibiting the negative feedback of estrogen on the hypothalamus, it will stimulate the production of GnRH (gonadotropin-releasing hormone), increasing the production of FSH in the pituitary gland itself. The production of FSH will result, at the level of the ovary, in the increase of the secretion of sex hormones, which will allow the correct course of folliculogenesis. This control also acts on the ovarian follicle contained in the ovary, which it will stimulate.

How does Clomid work?

The therapeutic efficacy of clomiphene citrate contained in Clomid is associated with its dual activity, both mildly estrogenic and anti-estrogenic.

This particular biological characteristic allows the aforementioned active principle to bind to estrogen receptors present at the hypothalamic and pituitary level with high affinity, inhibiting the negative feedback normally exerted by estrogens.

In this way the GnRH axis – Gonadotropins (FSH and LH) – estrogens, remains active, guaranteeing follicular maturation and subsequent ovulation, thus facilitating the establishment of a pregnancy.

How does Clomid work to get pregnant?

Ovarian stimulation cycles should last 5 days, with the daily intake of one tablet. In the event of ovulatory failure, the same cycle could be repeated 2 more times with higher dosages and under strict medical supervision. Each phase of the therapy, from the definition of the dosage to the end of the cycle, should be supervised by your doctor.

How long does Clomid stay in your system?

The important efficacy of Clomid treatment lies in the advantageous pharmacokinetic properties of clomiphene citrate, which allow the patient to take the tablet orally, and the active principle to be easily absorbed at the intestinal level, and thus distributed in the circulation, to be eliminated afterwards a half-life of about 5 days mainly with feces.

What are the side effects of Clomid?

The appearance of side effects on the visual apparatus, fogginess and opacification, could make the driving of vehicles or the use of machinery dangerous.

Despite the short duration of the treatment can significantly reduce the risk of side effects related to the use of clomiphene citrate, it is possible that therapy with Clomid may be associated with the appearance of:

For this reason it is essential to supervise a specialist doctor in defining the treatment and throughout the therapeutic process. Moreover, the intake of clomiphene citrate, even if indirectly, could determine a volumetric increase of ovarian and cyst ovaries as well as an ovarian hyperstimulation syndrome, such as to require the rapid suspension of therapy.

Statistical data describe how the incidence of twin pregnancies is more frequent in women undergoing ovarian stimulation.

At the moment, given also the short periods of treatment, active ingredients are not known, whose intake, together with that of clomiphene citrate, could determine the onset of clinically relevant interactions.

Contraindications

The use of Clomid is contraindicated in women with menometrorrhagia, changes in liver function, pregnancy and lactation or in case of hypersensitivity to the active substance or one of its excipients.

Clomid contains lactose therefore it is contraindicated in patients with galactose intolerance, lactase enzyme deficiency or glucose-galactose malabsorption.

The scientific literature reports numerous studies carried out on experimental animal models, in which the intake of clomiphene citrate has contributed to the appearance in the fetus of evident malformations, concentrated above all on the genital, gastrointestinal and nervous apparatus, such as to compromise the survival of the fetus. It is also useful to remember how the intake of this active ingredient during lactation could inhibit milk production.

Hi, I am a 22-year-old girl suffering from mild-grade micropolystic ovary syndrome, but I am not suffering from obesity, nor from acne, nor from insulin resistance, but I suffer from amenorrhea and hirsutism. If I receive Clomid (clomiphene citrate) the menstrual flow arrives. From hormonal dosages: I have estradiol 33, progesterone 0.76 and testosterone 53. During my life I have tried different treatments: Ethinylestradiol 2 for 24 days and Androcur for 10 days, with these the flow arrives, but I have had many side effects , Decadron every day and Aldactone every day, with these the flow does not arrive. What can I do?

I would say that in each of the treatments you did there was some good. Unfortunately, polycystic ovarian disease is an ugly beast that must be fought on several fronts.

I would use products that do not worsen the situation, such as Clomid and the like (with an anti-retention, antitestosteronic and antiandrogen action) without expecting miracles with regards to the hair that is already there, so, ultimately, more aesthetic treatments are needed. Do not despair, but know that the war will be long and in the end you will win!

After a year of trying to get pregnant we went to the gynecologist. It seems that I have polycystic ovary syndrome and that’s why they prescribed me Clomid. It went well for the first time (once a day for 5 days) and I got pregnant! I didn’t have any problems with side effects or discomfort.

Hello, I am 28 years old and have already had a pregnancy, trying to have a second child, I was prescribed clomid, to be taken “approximately from 5 days after the start of the cycle”, I started the first tablet on the 7th day, as I still had a period; later I discovered that Clomid has high risks of creating malformations to the future fetus if used during pregnancy and that it remains in circulation for some time, so I ask HELP, can I have created or what damage did I create if I were pregnant? Dear Ruth, Clomid is taken from the 3rd to the 7th day (starting from the first day of the menstrual flow). The drug is harmful if taken during a pregnancy, but you have taken the clomid outside of pregnancy. You should not worry.
