What is Diflucan?

Diflucan is an oral drug used in treatment of vaginitis and balanitis caused by Candida albicans, as well as cutaneous mycoses (tinea pedis, tinea corporis, tinea cruris and pityriasis versicolor).

What is Diflucan used for?

vaginitisIn adults, the treatment of candidiasis of the genital mucous membranes is carried out with tabets administered by mouth. Therapy includes a dosage of 150 mg administered in a single dose. For candidiasis affecting other mucous membranes (for example, mouth and pharynx) 50-100 mg per day are required for 7-14 days. In children the posology varies according to weight and age.

The treatment of tinea corporis and cruris and pityriasis versicolor usually involves 50 mg a day for 2-4 weeks, to last up to 6 in the case of tinea pedis. Doses should be reduced in the presence of severe impairment of kidney function.

How does Diflucan Work?

Fluconazole performs its activity by interfering with the synthesis of ergosterol. Ergosterol is a sterol that forms the plasma membrane of fungal cells; fluconazole is able to reduce its production by inhibiting a key enzyme involved in its synthesis: 14α-demethylase. By inhibiting this enzyme, there is an accumulation of ergosterol precursors inside the fungal cell.

The ergosterol precursors, when they reach too high levels in the cell, become toxic for the cell itself and cause alterations in the permeability of the cell membrane and in the functioning of the proteins contained in it. The fungal cell is thus condemned to certain death.

How Long Does Diflucan Stay in Your System?

Following oral administration, fluconazole is almost completely absorbed within two hours. Bioavailability is not significantly affected by gastric pH. The elimination half-life of fluconazole follows zero order kinetics. 10% of the excreted fraction has undergone metabolism while the remainder is eliminated as such with urine or sweat. Patients with kidney failure are at risk of overdoses. The half-life of the drug is estimated at mean 30 hours with the arch of 20-50 hours, being a subject to individual characteristics of patients.

Diflucan is metabolised mainly by the enzymes CYP3A4 and CYP2C19. In case of concomitant administration with benzodiazepines, a prolongation and an enhancement of the hypnotic effect of these drugs may occur, due to a reduction of their hepatic metabolism and to the persistence in circulation of high levels of active ingredient for a longer time. This interaction is well documented, for example, for tioconazole (eg Sporanox), fluconazole (eg Diflucan) and ketoconazole (Nizoral) when taken in conjunction with triazolam (eg Halcion) and alprazolam (eg Xanax and generics), and it results in a prolongation of the psychomotor effects, while in the case of chlordiazepoxide (Librium) there is a significant reduction, but not significant from the clinical point of view, of the drug clearance.

In the case of concomitant prescription of an azole antifungal and a benzodiazepine it is therefore necessary to evaluate the opportunity to reduce the dosage of the latter. The patient must be warned that prolonged sedation may occur and that he must consequently avoid getting behind the wheel or handling equipment that requires concentration.

Diflucan Dosage

Diflucan dose depends on the nature of the infection. Standard treatment for vaginal candidiasis involves taking one 150 mg tablet orally. Severe or systemic infections treatment starts with 200-400 mg on the first day followed by 100-200 mg daily for four weeks.

Prescribed daily dose for children depends on their weight and ranges between 3-12 mg per kilogram.

What are the side effects of Cipro?

Cipro belongs to the class of fluoroquinolones. Overall, these are generally well tolerated antibiotics. A review of 28 randomized, double-blind, controlled studies found that in 22 studies Cipro were similar to other comparative antibiotics in terms of incidence of undesirable effects and 5 were better tolerated.

Although it may slightly differentiate between incidences and types between one compound and another, the undesirable effects of Cipro are generally very similar and mainly involve the gastrointestinal system, the central nervous system, the skin and, to a lesser extent, the apparatus osteotendinous.

System Affected Symptoms
Gastro-Intestinal System At the gastrointestinal level the most frequent disorders are nausea, dyspepsia, vomiting, abdominal pain, diarrhea. Cases of pseudomembranous colitis have rarely been reported.
CNS Dizziness, headache, insomnia are the most common side effects of the central nervous system. Furthermore, there has rarely been reported impairment of taste and smell, visual disturbances (eg diplopia), tinnitus or temporary hearing loss, especially for high frequencies. What is Doxycycline?

Doxycycline is a broad-spectrum antibiotic that belongs to the tetracycline class of antibiotics. It’s used to treat various bacterial infections, including skin infections, respiratory infections, and sexually transmitted infections. Doxycycline works by inhibiting protein synthesis in bacteria, thereby stopping their growth and proliferation.

  • STIs – the dosage of Doxycycline varies from 100 mg to 200 mg per day for 10-14 days, depending on the severity of the disease.

Each patient takes the drug individually, but in any case, it should be at least 7-10 days. Otherwise, pathogenic microorganisms acquire resistance to antibiotic therapy (resistance to antibiotics develops, which threatens the appearance of super bacilli).

Important: Doxycycline is contraindicated in children under 12 because it binds calcium in the bones and teeth. As a result, the growth and development of the bone and dentoalveolar system is impaired – bone fragility and a condition called “tetracycline teeth”.

Doxycycline hyclate vs monohydrate

Doxycycline hyclate and doxycycline monohydrate are two forms of doxycycline. They are chemically similar and used to treat the same types of infections. The main difference between the two is in their formulation: doxycycline hyclate is more soluble in water, while doxycycline monohydrate is less likely to cause stomach side effects.

Can you drink on Doxycycline?

Interactions

Use of the drug during lactation:

  • In women who took 100 or 200 mg of doxycycline orally, the concentration in breast milk was about 0.8 mg/L during 2 and 3 hours and about 0.4 mg/L during 4.6 and 24 hours.
  • The highest concentration of doxycycline in breast milk (0.96 mg/L) was observed between 2 and 4 hours after 100 mg and 1.8 mg/L after 200 mg. Various studies have shown a peak concentration of doxycycline in breast milk after 5-7 hours of mother’s intake: 0.6 mg/L after 100 mg and 1.1 mg after 200 mg.
  • It has been estimated that the infant receives 6% of the mother’s dose, calculated by her weight. It is unknown whether such a dose of doxycycline in a mother’s milk is sufficient to cause tooth staining or inhibit bone growth, but such effects are theoretically possible.

Contraindications

Doxycycline antibiotic is not prescribed in:

  • Hypersensitivity to the active ingredient or excipients, as well as to tetracycline group medicines;
  • Pregnancy, in each trimester (the medicine penetrates the placenta, which leads to impaired growth of skeletal bones and teeth in babies);
  • Breastfeeding (the drug penetrates the mother’s milk);
  • Leukopenia
  • Porphyria;
  • Severe hepatic insufficiency.

The drug is not used to treat adolescents under 12 years of age weighing less than 45 kg.

Effect on fertility (male and female):

The effects of doxycycline administration on male mice at puberty were studied. Therapeutic doses of 1.4, 2.8, and 5.6 mg/kg/day were administered. A decrease in the anogenital distance at all doses, a decrease in testicular weight at low doses, and a decrease in sperm concentration at medium and high doses were found. There was a decrease in the thickness of the epithelium but not in the diameter of the ejaculatory duct, an increase in the number of apoptotic cells, and a decrease in the number of spermatids.

Doxycycline (tetracycline) penetrates rapidly into human sperm and can affect fertility by inhibiting sperm production. One study found that a couple who had spontaneous miscarriages in previous pregnancies showed better results in subsequent pregnancies when prescribed a course of doxycycline. According to unconfirmed data, this can be explained by the effect of doxycycline on the pathogens of urogenital infections that led to failures in previous pregnancies.

Overdose

Information on animal studies:

Experimental studies on mice, rats, rabbits, and monkeys have not shown an increase in the incidence of congenital malformations in the offspring. Although no data suggests such a risk, it is believed that doxycycline, like other tetracyclines, may cause tooth staining and inhibition of bone growth, especially of the fibula, and more often in premature infants.
